Three years, 5. 5 million patients healthier
We’re proud to announce that the Adult Immunization Best Practices Learning Collaborative, a joint effort between AMGA Foundation, Optum, and Pfizer, has resulted in more than 5 million influenza and pneumococcal vaccinations adminis-
tered or documented over a three-year period.
Launched in 2015 with a successful pilot program, the Collaborative has
engaged a total of 46 medical groups and health systems with a total of approx-
imately 5. 5 million vaccinations administered or documented as a result.

The Together 2 Goal® campaign is thrilled to announce Plank
Mentors. Plank Mentors are individuals who have excelled at
implementing one of 11 evidence-based care processes (or
“campaign planks”) to improve diabetes care within their organization. They are available to fellow campaign participants to
answer questions, offer expertise, and share lessons learned
on a given campaign plank in a one-on-one setting. Our 11
