Campaign exceeds goal of improving
diabetes care for 1 million people
AMGA Foundation’s Together 2 Goal® campaign has officially reached (and exceeded!) its goal of improving care for 1 million people with Type 2 diabetes. Through this national campaign, more than 150 AMGA medical groups and health systems have
implemented one or more evidence-based care processes (called “campaign
planks”) to drive improvement. As a result, 1,082,000 people have experienced improved diabetes care over the first three years of the campaign.
Through the remainder of the campaign, which concludes March 31,
2021, participating groups will have time to continue to hardwire improvements and new groups will have the opportunity to learn from the great
work that’s already been done. All organizations can leverage the latest
resources, including T2G Plank Mentors, interactive campaign planks,
webinars, and more to help them reach their diabetes improvement goals.

Announcing Cardiovascular Disease (CVD) Case Studies
We are thrilled to share that the case studies from the Together 2 Goal®
Innovator Track Cardiovascular Disease (CVD) Cohort are now available at
together2goal.org/successes/index_successes.html. From May 2018 to
May 2019, the 12 medical groups and health systems that participated in
this cohort explored how providers can better identify and manage CVD risk
among their patients with Type 2 diabetes. Participants reported quarterly
on six CVD-related measures relating to non-tobacco users, daily aspirin or
anti-platelet agent use, and lipid management for secondary prevention. Over
the course of this initiative, participants saw improvements in all six measures
using a variety of interventions. In the case studies, organizations provide
detailed insight into their interventions, outcomes, and lessons learned.
