Dear AMGA Foundation Participant and Donor,
I am proud of what we have accomplished in 2018. We completed
our second year of the national Together 2 Goal® campaign,
reaching 750,000 people with Type 2 diabetes. We launched the
Innovator Track to tackle cardiovascular disease and eye care for
patients with Type 2 diabetes. We also launched the Together 2
Goal® Diabetes Bundle Best Practices Learning Collaborative,
which aims to use advanced data collection and predictive
analytics to accelerate progress in diabetes care. Approximately 2 million vaccines were
documented or administered by groups participating in the Adult Immunizations Best
Practices Learning Collaborative, with a total of over 4 million vaccinations since 2015.
We kicked off our Obesity Care Model Collaborative to engage groups from across the
country to identify best practices for treating patients with obesity.
Already, with your support, we have reached 26 million patients, and are making progress
toward our goal of creating a healthier America for 100 million patients by 2025.
In this report, you will read more about the value that AMGA Foundation provides to
medical groups, health systems, and patients throughout the U.S. You will learn from
new and sustaining donors why they give and why they encourage others to support the
measurable results that AMGA Foundation’s programs yield. Together, we can change the
course of chronic and preventable diseases in the U.S.
